Focus stays with a target 'til it either dies, you leave the zone, or you set another focus. You can tell if somehting is focused cause its portrait will have a glow around it. to set it you just type
/focus
while it is targeted (or you can make a macro that is just /focus to do the same thing)
then no matter what you target, the focus is always the focus (so say your shakling a horse in the first pulls of kara, you target it and type /focus, its portrait highlights. Now you can target the tank, throw a renew, then soon as pull is made you can just use a macro like
/cast [target=focus] Shakle Undead
you will still be targeting the tank, but the spell will cast on the focused target. once the focus is dead, you have no focus again, and that macro will do nothing.
you can change your focus, just my setting it to a dif mob, like if you target the skull and type /focus, then see that its the skull not the star you can target the star and type /focus and it will change your focus to that mob.
BTW Focus serves no purpose other than macros.
