Nobody's perfect
If you play WoW long enough, you're bound to make mistakes. Whether it's accidentally picking up the wrong piece of loot off of a boss or pulling a bunch of elites before your group is ready, everyone messes up once in awhile. While I normally make a habit of doing research in order to avoid such things, the other day I made a mistake that only affected myself, but it was pretty annoying nonetheless.I had been spending a lot of money power-leveling professions for a few of my alts when I got lucky and picked up a couple upgrades for my main character. I was low on funds and needed to grab some enchants and gems quickly so that I'd be prepared for an upcoming raid. With little time to farm, I hit the auction house and looked for some deals. When I noticed the low price of Nexus Crystals, I was reminded that my enchanter alt could transform those into Small Prismatic Shards and then again into Large Prismatic Shards. After double-checking the prices for the various materials, I concluded that I would not only save a good deal of money, but I'd make a bundle by selling extra shards. I bought all of the Nexus Crystals on the auction house and shipped them off to my enchanter. Gleefully imagining the piles of money I'd soon be raking into my coffers, I switched characters and converted the first crystal into a shard. My schemes where short-lived, sadly, when the realization that the conversion of Nexus Crystals has a cooldown, much like an alchemist's transmutes. Oh well, at least I'll have a nice stockpile of shards after about a month or so.

Let's see...
-Accidently DE'ing my weapon instead of enchanting it (luckily it was in the mid 30s, not too big of a deal).
-Accidently buying a high priced BOP item from a vendor that's completely useless to my character (I've done it too many times to remember them all).
-Splitting a single item from a stack of gems/ore/enchanting mats/etc. then putting it directly into a letter for COD, not realizing I'm sending the whole stack.
I'm sure there's more I can't remember. My brain is trying to save me from it's own stupidity I suppose.

I crafted a Crown of the Sea Witch back when it was BoP thinking it was BoE, I didnt have a miner alt and had to buy the khorium (back then it was still pretty expensive), I even bought the enchant and blue gems for it >.

Various raid leader mistakes: (Not all mine)
Forgetting to tell people to stand on the horses ar*e at attumen = wipe
Forgetting to tell the pally healer to use BoSacrifice on maiden.... Wipe
No stealth characters available? Trying to peek around a corner to check it is safe.... Guess what? Actually we didn't wipe on that one but it was close
Completely forgetting you chose not to kill that one mob because it was a waste of time and then running into it after a wipe.
Everyone makes mistakes and the only way to make those mistakes worth something is to learn from them. Anyone who says they have never caused a wipe in WoW is lying only to themselves as no-one else believes them.

First time i ever went into SM, killed the mage boss and hypnotic blade drops me, then a lvl 36 rogue, thought oh great a blue dagger need that, stupid me our healing priest also needed it and i gave him a piece of my mind only to find out it wasnt really any use to me

Wiping the whole group on the first pull in Auchenai crypta - I ventured too close, thinking I'd just FFF one of the mobs .. guess I should have waited for the hunter to MD. How embarassing.
Another rather funny one - couple of days ago, I was running SL with friends, we're at the last pull before Grandmaster Vorpil, rogue saps, I pull, rogue unstealths too close to the boss and pulls him. We all start sprinting towards the exit, then get teleported back to Vorpil's plattform. That was a quick and painful death, but we were all laughing our a** off in Vent.

First time in COT I (Old Hillsbrad). I had to convince my guild to go there non-heroic, because I was the only one who hadn't done it yet. So we finally got a group together, ran through it without problems...boss down, everything fine. Everything? Unfortuately I didn't realize that I had to talk to Thrall when he was still in that prison to hand in my quest. I just didn't read the quest description. So we had to go again...that was by far the most embarrassing WoW moment for me.

Accidentally hitting Commanding Shout along the final trench in Gnomer.
Vendoring my first rare shield and not noticing until I was called up to tank SM. Got there "hey, where's my shield?"
Dropping about 300g on new rogue gear at around level 58, then getting to Outland and replacing it with questing gear.
Getting too close to the cages in the BF second boss room ... twice.
Picking up enchanting on my warlock. Dropping enchanting and picking up engineering instead. Got bored, picked up enchanting again. Got bored, picked up engineering ... finally dropped engineering and picked up enchanting, which I now have at 300.
In BRD, Searing Blade drops. I misread the text, and think it's a chance for +fire damage, not a fire blast proc. Roll need with my warlock against the rogue in the party. /boot.

Trying to corner a market (flooding the AH with a certain ware at high prices and buying up all cheap competitors) is always a mistake. There are always dumb***es who do not realize that high prices are good for everyone and flood the market with cheap wares as soon as the prices go up. I've seen this a trillion times. There are three, maybe four people regularly selling item X for price Y in the AH. Then, someone new shows up and sells for Y-1 gold. I don't know why these guys do not realize that that there are silver and even copper coins available to undercut your competitors. If three guys of this kind show up at the same time, you can imagine that prices almost instantly drop by 50%. So, if prices go down, do not buy. Instead, withdraw from the market for a few weeks and wait until the jerks have earned their epic and are gone. I made this mistake too often.

I was getting a run through ST from a warrior friend of mine and we're on the "Prophet" boss dude. And the warrior tells me to stay just out of the room because he has Mind Control and he would 1 shot me....actually he would 1/4 shot me...Whan I realized this I got scared so I fled down the hall. He tells me to come back and loot the room and I couldn't loot the boss. I missed out on all the caster loots on the boss because I didn't stay EXACTLY where he told me to stay. T_T *is stupid*

Here's two of my biggest mistakes that I've made:
-Using Divine Intervention on a Druid instead of the Priest in ST, after he blew his Rebirth cooldown, while being in front of Shade of Eranikus after he deaggroed... (Yea, it's impending death either way for him.)
-Accidentally getting too close to the third boss (The hunter guy with his pet boss) in Heroic UB and aggroing him, we still managed to down him though.

One of my guild mates has been hitting Kara for months with his rogue, we've cleared everything repeatedly and he's got just about everything he could possibly want there except the emerald ripper from Moroes. After months it finally dropped for him the other night and one of our other enchanters gave him the mats to put mongoose on it right there. He opens his inventory, opens his enchanting window and thinks he's hit the button to enchant it... all we heard over vent was a sound of pure agony after he realized he hit the DE button instead of the enchant button.
In the end he got it restored but he'll never live this down.

My biggest gripe/mistake was gathering all these mats (pre-TBC) to make some of the high end tailoring items. Only to realize after making one of them that it was BOC (Bind on Craft). That was about 300 wasted gold seeing as a Rogue doesnt really want to wear some cloth priest gear.

I once pulled Drakkisath by shooting a rat at his feet with an arcane shot. This was back when UBRS was still 15 man, my guildies still havent let me live it down.
