I've never run in a group on my current server that didn't use the pass and discuss rules.The main thing is that for the good of the group (and sometimes the good of the guild, if you're on a guild run), it's better for someone who can actually use the loot to get it. If a neat healing trinket drops, I might want it for my shaman in case I go resto...but if it's an upgrade for the healer in the group (especially if it's not off the final boss), we will have a better chance of success after giving it to them than we would by using the in game need/greed roll. And making people discuss it first also means you can check out people's equipment, and see if it's really an upgrade-most of the time people are being honest, but with those you don't know, it never hurts to be sure.
