Moonkin form was introduced in 1.8 with 360% armour.
it never had 450%, when bears was nerfed downwards from 450% Moonkins were buffed upwards from 360%
Melee on mana came with patch 2.0.1 or TBC as did the 150%AP for level which is when moonkin form was added to FAP weapons (well okay weapon as only one existed in Classic WoW).
