It actually looks like an older screenshot (based on the Omen window in the upper right), but I'll give it a shot:

Unit frames: X-Perl
Action bars: Could be one of a handful of bar addons. (I like Dominos myself.) Just choose one with ButtonFacade support to make the buttons look that way.
Upper bar/menu: Titan Panel I'm guessing.
Lower background: SunnArt (as mentioned)
Minimap: SexyMap will do the trick, although it may not have that exact border.
Buffs (lower right): ElkBuffBars
Threat meter (upper right): Omen

I think that about covers it.
