The only instance I haven't tanked / been to is The Old Kingdom, nor have I tanked The Old Kingdom, Azol'Nerub, and Halls of Lightning Heroic Modes, but I have to say that so far all of the instances are very fun and unique in their own way.

I think Utgarde Keep and Gundrak are the easiest Heroics when you know how to do everything correctly. I never have that sinking feeling when walking into those. Nexus is pretty easy too, but on Keri I sometimes wonder why my healers like to let Immense Cold stack. So I don't get the Achievement? Cry.

As for the hardest, Utgarde Pinnacle's third Boss, Skadi, is immensely annoying, but not exceptionally difficult. The Oculus is also a fan favorite for the hard factor, but I think that's mostly due to the unforgiving fourth boss and the trouble people seem to have with the idea that you need to keep moving on the third boss. I thought Halls of Stone was fairly challenging with the adds spawning, but I also had an under-geared healer for that run.
