At about 73 I went exploring in Sholazzar, working my way up the river fishing the nettlefish schools and mining saronite. Wound up in the lost lands staring hungrily at a rich saronite node guarded by a couple of lvl 76 undead. Figured what the hell and went for it. Killed em and one dropped the book. Didn't realize what it was until I'd finished mining the node and had scampered back to safety. Friend of mine upon hearing this bee lined to Sholazzar and proceeded to get it on her 1st kill so...../shrug
