As just mentioned, the highest rank pvpers were very rarely the most skilled pvpers, getting to the highest spot was simply a matter of time. You needed to be in BG's 24/7 and you'd often find the top ten players on any server would all end up communicating with each other and organising when each of them would acquire the rank.

It was just a highly organised farmfest for people who simply had more freetime than the rest of us.
