I actually caused this in a 25-man PUG I was in, I didn't say anything and I guess my vent updated and had lost all its settings (my mic is really loud so I have to turn it down). I said something once and supposedly everyone had to throw down their headset in the middle of the boss fight cause I made their ears bleed. I just kept quiet after that.
