My worst experience was the fabled and ever annoying Shade of Aran in Karazhan. When it first was released, our guild was the fastest to progress. Unfortunately our efforts were halted as server-first due to my moving during Flame Wreath. We wiped, and my GL said it's okay, try again.

However, nobody was paying ANY attention to their DBM or listening to what the GM said, so we wiped 5 more times until the GL said "Okay, that's it. You all fail."

So after ganking the bank, the guild leader disbanded the guild and was soon banned afterwards for buying gold from Chinese gold farmers. Making fun of that GM in Trade chat became the Shattered Halls phenomenon.
