Forgetting to hit the "Need" button
Kalon at ThinkTank touches on a subject near and dear to my heart -- missing out on loot upgrades because you forget what you're supposed to want. While on an Ulduar-10 run, he passed on the Kologarn drop Mark of the Unyielding, which is close to being the best-in-slot neck for bear tanks, for one good reason (the other tank raids more than he does at the moment) and one bad one (he couldn't remember just how good the piece really is). Particularly galling, he notes, is that he recently wrote an excellent guide to tanking upgrades in Ulduar in which Mark of the Unyielding was singled out for especial commentary. Ouch.For myself, I don't think anything quite approaches the sheer agony of having accidentally passed on Idol of Worship from Grand Widow Faerlina just because I wasn't paying attention at the time. I don't do much cat DPS, but losing the best-in-slot idol directly affecting the attack (Rip) that contributes the largest share of feral DPS is...again with the ouch. I still ask myself what was so amazingly enthralling about tanking Maexxna trash that night that I could have zoned out to that degree, and -- naturally! -- the idol never dropped again. While I guess it's possible that there are players out there who are practically perfect in every way and have never missed a drop, I have the sneaking suspicion that most of us still find a reason to kick ourselves.

My guild was working on ZA when the TBC raid nerfs were released. After the nerfs we started doing T6 content. Normally I understood the difference between T4 and T6 gloves, I was just very sleepy during our first Hyjal run, so when T6 dropped within a week I got my T4 I didn't roll on it because I already had my tier gloves.

don't feel bad Allison i accidentally sharded my Idol of Worship, only thing that's worse than forgetting to loot it is looting then sharding the new thing over your old pice

For DEing I always roll greed in PuG Heroic's never need...and make sure to let everyone know at the beginning of the run to need if they need something I don't know why someone would be rolling need for DE. Also in a 25 man raid...why would you not use master looter...no matter how much you trust your guildies, it's faster and allows for discussion before distributing loot.

I have the opposite problem. I feel compelled to check every single item drop against my current gear. Whether it's cloth (I'm a priest) or not. Even greens from normal dungeons I still have to take a glance at before I feel comfortable hitting the greed button.

And this is exactly why I felt so stupid. We do use loot master. I didn't miss out on rolling - rolling in this case is just expressing interest. The other tank is one of my good friends, and we had a discussion on vent about it - and both of us were basically telling the other to take it.
And I totally spaced on exactly how good it was overall. I have rating buster, and I wrote the damn analysis of how good it was and published it, and I STILL messed it up.
It's not a big deal, but boy did I feel stupid.

This item (along with other naxx idols, sigils, and librams) can now be purchased at the Emblem of Valor Quartermaster for 25 Emblems of Valor.

Yup I've done that many times. Also been called out on it too. There is nothing like hitting greed and finding out it was way better. My problem is that I do it out of habit.

This may not count but the other week in Naxx ten, Loatheb's Shadow came up. The loot master won the roll and then accidently gave it to another toon. Funny thing is it was my third time with him and it was the only drop he REALLY wanted from in there.

Id be lucky if I ever got an orange or red item let alone an purple item. I guess that makes me an in game loser.

Missing on loot because you accidently rolled need is bad.
But winning the perfect staff for your mage and afterwards trying to enchant it and accidently hitting the disenchant button is even worse. Believe me, I quite playing after that for several weeks because I was busy kicking myself in the head.

I've got an addon that tells me instantly on mouseover how the item compares to what I currently have slotted. No loot tables for me to remember, which is especially handy on my alts.

I've missed out on upgrades before, but for a different reason. I'm usually so worried about offending someone, I'll end up hitting Greed at most. There's some stuff I've only got because the group leader went "what do you have? Okay, Need that." or "you're the only person that can use that, Need it"
