It absolutely amazes me that Blizzard - after nearly 5 years of handling WoW - continues to fail at simply QC procedures when dealing with maintenance and hardware upgrades. There is no way anything that affects this many servers should have been allowed to go this wrong. I have no sympathy for them whatsoever - they have more than enough resources to avoid these problems, yet they clearly don't feel like investing the money and/or manpower needed to properly handle it.

As far as I know Blizz don't do all the upgrades themselves, I would imagine there would be a fair bit of outsourcing done. For example, about a year ago (I think) and at some stages earlier, server maintenance was far better than it is now, in fact, there were times when some servers didn't even need to go down for the maintenance, however when the Blizz/Activision takeover occured, everything went to crap, and I am only guessing this would be because of outsourcing.I have a friend that used to do the Maintenance back in the Vanilla WoW days and he said that even back then, the original guys had left and no one really knows how the systems work, its kind of like a magic box they just keep poking so it keeps going.
