I believe the Tram zone itself is actually situated underground just east of Ironforge. Some map addons (like Carbonite) show it like that.
The Tram itself is somewhat weird really; it goes underwater in one moment, yet there is no sea between IF and Stormwind, in fact, no big water body like that. Which leads me to theory that the tram was originally supposed to connect Ironforge with something in the east, and the water above Tram tunnel is a lake in Loch Modan. When they decided to connect IF and SW, they just took the finished tram zone and changed the exit, leaving everything else as it was. Which is why Tram goes from west to east.
