Gear and content go together by design, but it isn't *necessary* for the design to go that way.
There's no reason why content has to scale such that you *have* to have the gear from heroics to run raids and have to have the gear from Naxx to do Ulduar, any more than there's a reason why Ghouls are level 80 in Northrend, level 60 in the Plaguelands and level 10 outside Silvermoon.
I'm increasingly of the opinion that what Blizzard should do to make raiding more "accessible" is - rather than having 10-man "normal" and 25-man "heroic" modes, both of which represent parallel lines of progression, have 10-mans drop and require equivalent gear to Heroic dungeons. That way they'd be accessible to people who want to do the content, but don't want to grind until they meet the essentially arbitrary gear requirements.
