Could someone please explain in simple terms how LOH works anymore?

I had a couple "Oh Sh*t" moments last night and I mashed LOH and nothing happened. Mousing over LOH on my task bar the text doesn't say anything about prerequisites - LOH won't happen if you do this, this, or this. It does say that if I use LOH on myself a lot of things won't work to protect me for 1.5 minutes after that.

Adding insult to injury is the fact that LOH isn't grayed out like it's on a cooldown or something is preventing it from working. It's bright, shiney, ready to be used, but - Doesn't work.
