Khadgar explains that the skull is linked to the original Dark Portal as Gul'dan helped make it. I believe in the book, Beyond the Dark Portal, Ner'zhul and Teron need the skull in order to help reopen and expand the rift between Draenor and Azeroth. So after that, Ner'zhul does not need the skull but he holds onto it really to gloat to his dead apprentice.
Khadgar, however, needs the skull due to its link to the portal to try to perminantly shut the portal down. He also needs the book of Medivh since it contains the spell work about the portal, (which also why Ner'zhul needed teh book. Medivh was the only one who really knew how to open portals; the magus just gave Gul'dan the knowledge to open one to Azeroth.)
