Just like on job apps, they ask you if you'd ever been fired from a job before. If you say yes, but explain it maturely and demonstrate how you learned from it it's actually more of a positive than a negative (in my experience). Of course, depending on the offense, for example there's no way to put a positive spin on say, flipping out and jumping on your desk, stripping naked and screaming "WELL BLOW ME DOWN MATEYS!"
Whether or not they care about his previous offense, they SHOULD care about the fact that he lied about it. That's my biggest issue here. Not that he left for five days without telling, that he came back and lied about who he was. First of all, if he was active in the guild at all before, how long does he think he can cover it up?
Secondly, and most importantly, what else is he covering up? The guild leaders need to know. A person who got a guild invite based on lies is bad news bears. You can send it anonymously (though yeah, if you're the only person he's told, he'll probably know it's you) but you'd be doing your guild its best interest to tell them.

We had a situation where we kicked a guy for being a whiny pain in the butt. After a number of guilds (other guilds told us their dealings with him) he disappeared. A few days later we got an app on our guild website (we use guildlaunch) and it was a similar name to the guy we had kicked. I thought something was fishy but the GL had invited him before I could say anything. (I am an officer btw.) I did a little detective work and his dungeon achievements matched up to mine. Needless to say I told the GL that it was this guy. The GL asked him and it was the player we had kicked before. The GL let him stay since he did fess up to lying.(The GL is a softy and gave him an undeserved second chance.) We ended up kicking him like 2 weeks later for being a whiny pain in the butt.
Anyone willing to lie about their guild history has something to hide and it usually isn't something good.
