10. Alone in the Darkness
I've previously described the Yogg + 0 encounter as "a screaming, gibbering nightmare," and I stand by that comment proudly.
Yogg with no keepers up simply does not permit mistakes. Even if you do it with the benefit of heroic ICC gear, your raid has to play perfectly to avoid the seemingly endless array of ways to get yourself killed -- or worse, driven mad. Yogg can easily RNG even amazing players into a sliver of Sanity entering phase 3, and there is literally nothing you can do about it. The only way to survive is -- again -- to play perfectly, because there's no way to make up for whatever you've lost. You giveth yourself a margin for error, and Yogg? He shall taketh away.
The Sanity mechanic alone guarantees that this encounter is never going to be a cakewalk for the length of Wrath, and I'm curious to see if it even gets easier in Cataclysm. While a few guildies have floated the idea of doing Yogg + 0 at 85 to farm Mimiron's Heads (for people who, for some inexplicable reason, want a reminder of the nightmare that was Firefighter), not everyone's on board. I guess there are only so many times you can watch your raid disappear under a murderous pack of completely unkillable adds before you break out in a cold sweat at the thought of getting within 50 miles of Ulduar again. If you watch Stars' world-first kill, and more particularly the final few seconds where they sacrifice most of the raid for one or two more seconds' worth of damage, you'll get a sense of just how desperate phase 3 can get.
More than a year after Ulduar's release, a lot of servers still don't have a Yogg + 0 kill (and of those that do, most have just one) -- and it took the world's best guilds the better part of three months to manage it. Many raiders think the only encounter in Wrath that equals the difficulty of Alone in the Darkness is ... well, you'll see later on in our list.

9. Higher Learning
While this is a great achievement for lore nerds, it's an RNG-infested pain in the ass to complete. Your task, should you choose to accept it, is to find a series of eight disappearing and reappearing books all over the city of Dalaran, with the eventual goal of reading the complete Schools of Arcane Magic series. For your troubles, you'll get this achievement and a neat little arcane pet. Of course, you're also likely to get an ordeal that may span months unless you're willing to arrange your schedule around the books' 3-4 hour respawns, or be online as soon as the servers come up after maintenance. While this is certainly the easiest way to guarantee that you'll find a book in the spawn area, occasionally things like gainful employment stand in the way of logging on Tuesday or Wednesday afternoon.
Easily the most frustrating thing about this achievement is the high chance of getting a "dummy" book that's not part of the series. From a lore perspective, it's fantastic and wonderfully appropriate that you never know what book you'll get in a city steeped in magic, and many of them are actually interesting to read.
From a game mechanics perspective -- and especially if it's that one last Arcane Magic book you need that resolutely refuses to spawn -- it's wait after wait after boring and usually disappointing wait.
8. Hero of Shattrath
The Aldor and the Scryers hate each other. How do you know? Every time the Aldor decide to like you just a little bit more, the Scryers decide the opposite, and vice versa. The only way to get this achievement is to reach exalted with one faction, then turn around and start the long, painful, horrible grind to from hated to exalted with the other. Because their respective hand-ins are no longer very common on the auction house, you'll probably have to get most of them yourself.

7. Accomplished Angler
From our earlier article spotlighting three of the most evil achievements:
Accomplished Angler is so stuffed with RNG that you could theoretically get the whole meta-achievement in the space of a single day -- or, like me, you could be slogging away at it more than a year into Wrath.
I debated whether to split up all the horrifying RNG achievements required for the fishing meta, but there are so damn many of them that it just feels more efficient to group them here. A lot of Accomplished Angler is really, really fun, especially for people who genuinely liked fishing even when the more reasonable among us would admit there wasn't much point to it. But for all those of us still chasing the "Salty" title, there's a depressing amount of luck involved in getting the meta. At this point, I've caught tens of thousands of fish without once seeing a rare needed for One That Didn't Get Away or getting a Sea Turtle during the year I've fished up countless pools' worth of Fish Feasts.
As an aside, I rather enjoyed our commenter Grovinofdarkhour's description of One That Didn't Get Away: If achievements were people, that one would be a dirty rotten son of a bitch that kicks cats and pushes old ladies. Testify!

6. Less-rabi
Either you'll nail this achievement on the first try, or (more likely) you'll spend the better part of an evening resetting the fight and trying over .. .and over ... and over ... -- The OverAchiever: Return of the Glory of the Hero.
This is, without question, the most despised achievement required for the Glory of the Hero meta now that Oculus has been clobbered by the nerf bat. Certain lucky souls out there may get it entirely by accident if they land a very high DPS group with lots of interrupts through the Dungeon Finder, but for the rest of us, Less-rabi is much less forgiving (unless you accidentally hit Bash at exactly the right moment and get it by accident).
Your goal? Kill Moorabi without allowing him to transform. The horrible part is that his transform cast speeds up as his health drops, becoming virtually instantaneous below 20%. Below 50%, he'll try casting it every 5 seconds, and unless you have better reflexes than Peter Parker, you're virtually certain to miss a cast. Oh, and Moorabi also has an irritating knack for using Quake at inconvenient times, stunning the group and allowing him to transform with impunity. Even if you have an elemental shaman with Reverberation (easily the best way to do the fight), the unpredictable stun can eighty-six an otherwise perfect attempt.
In some ways, this achievement is even more painful nowadays given much higher average group DPS; it's absurdly easy to kill Moorabi by accident trying to reset the fight because he got a transform off. I've seen a lot of players walk away from this fight frustrated and angry because it's the last achievement they need for the meta, and with so many players in tier 9 these days, it can be next to impossible to reset the fight before DoTs finish him off.

I finally got my Accomplished Angler title last Sunday.
This is after nearing two years of painful fishing, trying to complete the retroactive achievements as soon as the beta went live and non-NDA. After over 32 weeks of trying and failing to catch 40 Tastyfish before the gank-happy competitors. After nearly 20 hours fishing for the One That Didn't Get Away. After fishing in pools rather than Wintergrasp for Fish Feasts in the hopes of getting a Sea Turtle (which isn't technically even required). And, most importantly, after hours of sitting at that stupid fountain watching people all around me get their achievements as I held out for that last gold coin.
I hate Accomplished Angler. I wear my Salty title with an exhausted pride.

Heroic Arthas *will* be making an appearance in our top five, but if I put in all the heroic ICC fights that I want to drag behind a barn and kill with an axe, the list would be overrun with them. Now that you mention, I think a series on the most difficult or horrible raiding achievements would be really fun to do.
For what it's worth, my nominations for the 5 worst heroic ICC fights:
1. Arthas. This is a given.
2. Putricide. VIOLENT hatred for this fight in my guild.
3. Sindragosa. There are very few words to express how much I hate Sindragosa even on normal and utterly despise the Unchained Magic mechanic. Sitting there doing nothing for 90% of the fight with that f&%*#ing debuff is not fun.
4. Deathwhisper. So easy on normal, and such a nightmare otherwise.
5. Blood Princes. Actually, if you have a melee-heavy raid, this is every bit as bad as Sindragosa. Watching a good part of your raid literally DO NOTHING for heroic boss fights is just agony.

As melee, I personally hate blood princes with a fiery burning passion. "Oh look, lets all run together instead of spiting up!" Sindragosa is pretty bad too, but not as much for me as say, a rogue or feral druid as I am a ret paladin. Wait... I just got tiny abom... Ah !@#$. I personally think putricide needs to be dragged behind a rock and beaten to death with his own gas vials. In a 25 man seeing those damn things is impossible.

I actually found Jurim two days in a row and got the credit on 2 different characters. First one was a warrior a few levels too low to solo him, so I ended up dead. While I waited for a guild mate to come help, I watched him as a ghost, and saw his path now stops quite a long way from the guards at the fork in the road.

Most of these are well-deserved. Frostbitten/Bloody Rare are nightmarish. Hero of Shattrath is a terrifying grind.
The only one I don't really agree with is Higher Learning, because you can just make it part of your routine to check all the spawn points whenever you happen to be in Dalaran, and you'll most likely get it after a month or so.

agreed, i would go around to all the spawn points whenever i was in Dal, i logged in the mornings before work, and i found what the higher learning channel* is on my server. i got extremely lucky i think because i was once doing dailies in icecrown when someone wrote on the higher learning channel that a book was available. i immediately hearthed and got the book, checked the other spawn points and immediately found two more books. as soon as you find one spawn point, even if it's a joke book, check absolutely every other spawn point because they usually spawn in pairs. got higher learning in about a week this way.
* for those maybe not aware, a lot of servers start their own channel so people can help each other out with spawn points. i just asked in trade a couple of times before someone knew what i was talking about. usually not that many people on it but it was extremely helpful that one time. and always make sure you callout spawn points too, in trade channel.

There are also addons and even chat channels dedicated to doing Higher Learning, making it a slow but doable achievement. I did this on 3 different characters on Lightbringer. It just required patience, a keen ear, and leaving toons in Dal so I could quickly get to a book when a real book spawned. (Sometimes I had time to get the same book on 2 or 3 of my 80's before it disappeared). So, while a challenge, I'm not sure this one merits the top 15.

You haven't begun to plumb the depths of evil that is Frostbitten.
Loque is cake. Aotona and King Krush both are huge and targetable from halfway across teh zone, and they both patrol right through the middle of the herb farming paths. If either of those mobs is ever up for more than 60 seconds, I would be shocked.
Jorfus doesn't just spawn near the Argent Tourney. He also spawn right outside ICC, and is targetable from the entrance, where raids are hanging out basically every second of every day.
Grocklar spawns in one of three spots, all of which make him targetable from the major horde camp in Grizzly hills.
Putridus is so large that when he spawns in Icecrown, your NPCScan will go off from anywhere in Dalaran. This is another mob that never stays alive for more than 2 minutes, tops.
I have a very definite feeling that i will be unable to finish this achievement until after Cataclysm launches.

Tell me about it. I've been trying to get Bloody/Frostbitten for nine months now. My first two kills were Jorfus and Aotona. King Krush would have been my third. But one day I zoned into Sholazar from WG in the middle of Krush's southeastern patrol path. I could have north or south. I chose to check the northern half of that path first then double back to get the south. When I got back south, my macro picked him up already in combat. And there, right next to the Skyreach pillar, was Krush, just dying at the hands of some alliance dog. After that, I found almost all the rest pretty quickly, within maybe two months, leaving just three to find. Another two months and I found Hildana Deathstealer and Tukemuth within an hour of each other. That left just.... King Krush. In the subsequent five months I've been doing a flyover of his patrol paths once or twice every day and I still haven't found him again. In the process I've tamed Loque'nahak, and then killed him seven times. I don't even get excited anymore when he pops up on Silver Dragon.
For Bloody Rare I found Jurim easily. It's Okrek that I can't find in Terokar. Him, plus Fulgorge the worm in Hellfire, and Speaker Mar'grom in Blade's Edge that I haven't found yet. I don't spend alot of time in Outland though.
For all the trouble these acheivement are, I think there should be a reward for getting the both of them. I think a title is in order. Due to the amount of camping these potentially require, perhaps "Camper [name]". Or maybe "[name], the big game hunter". Or my personal favorite, "[name], Nesingwary's Pride". I'd wear that title constantly.

You know, I'm really surprised. You didn't mention the WORST THING EVER about trying to get the achievement, Hero of Shattrath. Getting to Neutral with the faction that hates you.
You have to turn in over one thousand three hundred Dampscale Basilisk Eyes to get to neutral with the Scryers. Now you would think that all the basilisks would drop two eyes. You would be dead wrong. Most apparently don't HAVE eyes.
For the Draenei you have to turn in over one thousand three hundred Dreadfang Venom Sacs. Once again, you would assume that all dreadfang spiders would HAVE one. But once again, you would be dead wrong. Apparently they don't HAVE venom sacs. They just poisen you with their minds.
If can assume that one out of five kills will net you an eye or sac, that means you will have to kill at least 6500 mobs to get enough quest items. Add in the amount of mobs in the zone and then the respawn time...... Yeah. Have fun!

I can appreciate how difficult Hero of Shattrath is on an intellectual level, but I'd forgotten to include the bit about the basilisk eyes/venom sacs. I have absolutely no intention whatsoever of ever grinding to Aldor exalted from my current Scryer exalted. I may be crazy, but I'm not THAT crazy.

I did the grind from Aldor exalted to Scryer exalted for alchemy recipes in BC before the Achievement system was implemented...and I have considered doing the grind back. Being a Scryer just doesn't seem right as a Draenei.

Hero of Shattrath is one that I have, and was a pain in the butt, but not the worst by any stretch of the imagination. I was exalted with scryers just from questing and turning in stuff I picked up while questing. Going back at 80 and farming dreadfang venom sacks with roughly a 25-30% drop rate for hours on end made me want to pull out my hair, and I did it all for, as my husband puts it, leather working patterns I will never use. I am a completeist, and I am still sad that I started playing to late to get certain LW patterns that were available when Naxx was lvl 60. I'll be working on the Insane next, trying to get it done before cata comes out.
