My interpretation was that half-orc/half-ogres bred with other half-breeds and eventually gave rise to the Mok'Nathal, which are more of their own race now than a race of half-breeds. Thus, you can be a half-orc/half-ogre, but if your parents weren't Mok'Nathal, you aren't Mok'Nathal.

That's certainly possible (even potentially likely) but at present no one steps up to explain it, much less why the Mok'Nathal in Blade's Edge look like Mag'har only bigger, but Rexxar looks more like an ogre. Is he a further cross breed? Or is it just a case of Blizz not coming up with a unique model for the Mok'Nathal? As yet we don't really know.

Yeah, this is definitely an area of the lore that Blizzard needs to expand, or at the very least explain.
