We use a Loot Ratio system in my guild. #of hours raided/# of loot taken (i.e. 10 hours/2 pieces= 5). Whoever has the highest ratio gets the item in question. If no one wants it, then anyone who wants it as an offspec is open to roll (at no ratio cost). The nice thing about this system is it allows you to continue to gain ratio even if you are on the subs list.

That's basically epgp, a great system that we use in my guild. EPGP is a bit more savvy as it adjusts for slot and item level, which helps if you are changing loot tiers, but still do old content.

We just start a new list for the new tier, allowing a certain percentage of the ratio to rollover.
