Do Kezen and the Lost Isles appear on the world map so you can select them and view their maps?

No, not at all. In fact, when you're in those zones you can't zoom the map out.

I can understand that for Kezen... but can't you go back and visit the Lost Isles after?

Trust me, if you level a Goblin through those two zones it will make sense to you why you can't go back to either one.
