I can say from experience that the Earthquake issue was a known issue on the PTR. It stands to reason that many others are known issues that are not game-breaking, meaning they feel OK about releasing the patch.
As another example: for a time on the PTR, the Froststorm Breath from BM Hunters' Chimaera pets was usable against friendly players. This led to insane amounts of griefing, as people are generally asshats. But I have to say it was fun to be on a holy priest and keep those offending hunters focused so I could see who their targets were and preemptively heal them. But all-in-all, it made the PTR almost unplayable at the target dummies while this bug was active.
Long story short: they've known about these bugs since the PTR testing phase, but some they just haven't worked out yet.
