AFAIK, only Stormwind and Orgrimmar get attacked. NPCs provide escape portals to the city entrance (I think) before the event.

In the outside world, it seemed every zone has at least some elemental activity. Even Outland had elemental invaders on PTR, which makes little sense.

Its just SW, IF, TB, and Org, and even when they do get attacked, about 2 minutes before the attack happens ports appear throughout the city to the city entrance and the npcs all start yelling for civilians to evacuate and for level 80s to start putting up baracades(away from the impending attack)

They attack Thunder Bluff on the PTR as well.
