I doubt Outbreak's cooldown is going to cause too much of an issue, at least early in the expansion. Between actually being stuck with gear within a tier of what the mobs are designed for (so the mobs take longer to die), the necessity of setting up CC, and the likelihood of the healer needing time to recover mana, it's going to be up most pulls anyway.
Also remember, when fights are longer diseases will tend to pull ahead (or at least closer); you've usually got a full set of runes at the beginning of a fight, and once you've burned those they become more precious, slowing down how often you HS/DS/BB/whatever, while the diseases keep ticking away at the same rate. A lot of the reason diseaseless is pulling ahead on live is how rarely anything lives long enough for the diseases to tick the full duration.
