Based on everything I have read/seen I would say the timeline in WoW is basically this:Fiver years from the emergence (post binding with Arthas) of the Lich King. While in Northrend Westfall and SW was rebuilt, Lich King defeated, Trolls and Gnomes reclaimed their homelands.Coming home from Northrend we find elemental unrest over several months in "game" time and now the Shattering. What we see in patch 4.0.3a is the initial aftershock of the Shattering, where we all are still figuring out what happened, cleaning up the damage and stopping further destruction (Darkshore). The Cataclysm expansion will move the world forward a bit more, but more importantly retcon the Golblins and Worgen in (the area is currently phased to be basically deserted after the Forsaken invasion). Azshara is already reformed by Goblins in 4.0.3a.So patch 4.0.3a is like a big event with an almost real-time vibe to it. This is the weeks after Sept. 11, basically. It's really messy, but I think that's the best way to internalize all these changes.
