The in-game duel between Garrosh and Thrall would end whenever either of them reached 30% health, I believe. Sometimes Thrall was winning at the time, sometimes Garrosh. It depended entirely on when they randomly cast their spells. Sometimes Thrall would start spamming lightning bolts while Garrosh whiffed a Whirlwind at the other end of the arena. Sometimes Thrall would end up getting stuck melee attacking while being pummeled by the Whirlwind.

In the comic, it's clear that Thrall provoked Garrosh intentionally in order to put him in his place, and Thrall was easily winning the duel when the invasion began.
