Never been defined! I think it can safely be assumed that whoever she was, she died of the red pox before Garrosh went to Garadar, since she's never been mentioned.

Since it's never mentioned, at all, it's safe to assume there's a reason. It's well known that Grom was one slightly insane orc, and also unusually slim in build. Consider this, in combination with Garrosh's appearance as a HUGE orc. it is safe to assume that it's not artificial steroids in game here. My hypothesis is this:
Grom did one late night, on a whim, or possibly a dare, court a female Gronn. Garrosh is proof that Grom did unspoken deeds of both bravery and madness.
