An important point here:The target the tank initially has is not necessarily the target he'll stick with. Of course raid marks are there to easily and clearly define the kill order, and they should be used, but if they're not don't assume the target the tank has when you click him is the one he's going to stick with, especially if it's the pull. I'll frequently throw a ranged silence at a dps caster and maybe a few other attacks at secondary targets as I run in on other mobs to make sure they don't aggro on the healer, before I switch to the primary target and start my proper threat generation.
