"The Cataclysm" was the breaking of Azeroth. Patch 4.0.3 was called "The Shattering", this is when "the cataclysm" happened and the old world changed. The expansion is called "Cataclysm", this is the expansion with new races and increased level cap.
"The Cataclysm", ie the shattering patch, happened in November.
"Cataclysm", as in the expansion, was released in December.
I would say that "cataclysm went live" or "cataclysm came out" on 7th December, because this is when the expansion became available. The shattering was a patch, NOT an expansion. Other patches have changed the world (eg sunwell/quel danas).
