The flags and such don't directly affect you at all: they don't make it harder for you to continue on your way after rezzing, they don't debuff you or anything, they're just a way of mocking someone. Mocking falls completely outside of whether it's ganking or not. It doesn't affect gameplay. The way you respond to it just depends one whether you have thick skin or let some faceless person on an online video game "humiliate you" or get you mad by getting under your skin.
I agree that all of the things you listed are bad to varying degrees, but I would contend that 4 is part of a pvp server. It's a rather silly view to think that people will (or should) wait until it's convenient for the person being attacked to fight them.

Well, again, I agree in part. The flag thing is a measure of skin-thickness BUT it's part of the tool-kit of these people who are very childish in their approach to pvp. If/when it happens to me, I'm just laughing because it says way more about the person doing it than my "death." The KIND of person who has these in their tool-kit just doesn't have my respect.
As for when to attack - sorry that defines what is a honorable altercation by my standards. Anyone can pick someone else off while they are fighting someone else. I don't care what class they are. So what is accomplished in that moment? Unless you are on a pvp-centered quest that I frankly, don't understand the point. If the point is to chest-thump then, again, they have my pity, rather than respect.
And notice, in no way am I talking about rogues specifically. I get that they have to stealth and hit from behind to be successful. Any classes can be asses. X-p

I'm a shadow priest. My personal favorite? Find a lowbie. Psychic Scream. Cast Mind Sear on the lowbie. Play bowling.
Usually I don't kill the lowbie though. But that's just me. :P

As a rogue, I get lots of pleasure out of slipping up on somebody and sapping them. I'll then run a safe distance away and enjoy a good laugh as the target starts jumping around spamming AOE. Equally entertaining is tossing a lowbie a /wave then hitting stealth just to watch them do the OH #*%^ twitch as they're waiting for the death that never comes. No harm done I move on. Just one of the perks of the class.
I too have an unwritten set of rules I play by and I don't typically engage in PVP. I let questers quest and farmers farm. I'm the type that will often throw you a /wave or a /salute and continue on my way. If I notice somebody being harrassed or ganked, that's when I'll jump in and help. I don't camp and I don't look for the person at 30% health fighting an NPC. Just no honor in such actions.

With all the misconceptions and judgmental hate I feel a little bit like Jay and Silent Bob reading their movie's reviews. Luckily, like them, I'm just a fictional character. One who apparently feeds on the fallen and steals the souls of innocents for Satan.
But seriously...
That thread was made with the intention of promoting fun (this is a video game) by bringing back world PvP. Despite however deplorable the list may seem to some, it's inspired others to test drive a Rogue. The original thread was entitled "Griefer's Kit of Griefing" - I thought I was actually making the language less malicious. Griefing entails making someone's existence miserable. It is against the TOS and not enjoyable to much extent for either side. Ganking, in my mind, suggests killing an unsuspecting target for no apparent reason when the opportunity presents itself fullest. That is the role of a Rogue - it's a a stereotype that's been perpetuated by the game developers themselves. We are the shinobi stalking the shadows. And we have many tricks.
As for lacking honor by utilizing all advantages or not conforming to a third-party's arbitrary notions of honor, I present to you one of the many articles Google has on the subject: http://www.sirlin.net/articles/playing-to-win-part-1.html. That said, I believe everyone has their own rules of engagements and limits on what they find appropriate, but they're never going to align perfectly. In fact, I often use alternative methods as supplements to actual killing. Mind games can be far more fun. But "fair" is relative and subjective. Think Captain Jack Sparrow shrugging off his dirty tactics with one word, "Pirate."
Imagine a Mage accidentally flags for PvP when he kills a Crashin' Thrashin' Racer - they'll learn something about environmental awareness, and world PvP shortly thereafter. The next few minutes will bring inevitable intermittent Saps, perhaps an Engineering mind control, followed by an opportune killing blow during an NPC fight. If they resurrect for revenge against the Rogue responsible, the Rogue may teach him about Free Action Potions - and the Rogue might engrain the lesson with a Flag of Ownership.*
*All parties involved in this hypothetical interaction were within 5 levels of one another so as to avoid further QQ in subsequent comments to this website.
