I'm not sure if this would count as a cataclysm death really... I didn't have the expansion at the time, but I didn't log off when it came live. I decided it would be fun to go into hyjal on my ground mount and get the exploring achievement. I got pretty decently far into the zone, as far as the worgen infested pool in the middle, only then did I die after seeing waaaaaaay too many white 200 on my screen!
