For those who haven't been through the Plaguelands, you board a caravan and pick up people along the way in side quests. Completing the quests (or talking to someone who needs a ride) recruits them for the caravan. For each person you get, you can choose a buff. There's a ranged/melee damage proc, a spell damage proc, a chance to find a couple extra silver in mobs, an XP boost, a knockback proc, a heal after killing a mob and a run speed increase.
Or, if you want, you can do the Darrowshire quest chain. Pamela's Doll gets added to the caravan and the accompanying buff lets Pamela run around, following you, as a non-combat companion. There's no actual gain for game mechanics with the buff.
Of course, being the pragmatic player that I am, I went with the Pamela's Doll buff. You can't live a little kid alone with all those nasty ghosts lurking around.

I chose to have Pamela accompany me too. It just didn't seem right to leave a girl all alone in that deserted village when she doesn't even know that she's a ghost. In the past this has been a quest-chain without a positive resolution. At least now we can keep her company!
Until we realize how helpful that knock-back buff is, at any rate ;)
