With regards to the last story, when we were working on LK25, we were once at bout 50% in P3, with 2 people down, but it was going well. Someone called for an innervate, and the only available was that of the feral druid MTing the LK. She had once done it on putricide without taking a melee hit in caster form and thought it would work again, so upon the call she blew innervate then frantically spammed Bear Form - unfortunately LK got a hit in and the lack of armour provided the LK with a 200k hit on her. The OT was quick on the ball though and we continued into P3 ^^Also note that the MT was actually an extremely good player, she just happened to think it was possible if well times and it backfired a tad :P At the very least it was one of the funniest tank deaths I've seen.

Maybe I run with crazy druids (actually I'm pretty sure that's a verifiable fact), but have seen druid tanks do this many a time. Unlike your tank, they tend to wait for the boss to start some cast, or hit the other tank, or during some other mechanic where they won't take a hit. But I seem to recall a few Barkskin casts of Rebirth or Innervate while they still had the boss' attention. At least they had their pants on. If they were REALLY picking on me they'd have done without those too.
