The latter seems unlikely for a ground effect. People can walk in and out of it throughout it's duration; if it didn't check every tick it'd be... odd. You'll have damaged guy running away from the green poison, and still getting healed, or if he does run, he's still using up one of your three healing slots (but not getting healed), or something equally absurd.
