As far as a title-collecting title goes, I really like "the Entitled."
I'd love to see profession-related titles. They did it for Archaeology. I'd love to see it for the other professions.
Engineering - Tinker, Grand Tinker, Master Tinker, High Tinker (look out Mekkatorque!)
Blacksmithing - Smith, Master Smith, Master Swordmaker/Axemaker/etc.
First Aid - Nurse, Doctor, Surgeon
Fishing - Angler, Master Baiter
Jewelcrafting - Gemologist
Enchanting - the Enchanting, Enchanter
Tailor - Seamster/Seamstress, Tailor, Master Tailor

Would the profession titles be for just maxxing the profession out or perhaps a certain number of epic recipes collected/crafted (recipe collector here). It says a lot if you can collect every crafting recipe in the game. My guild is doing a bunch of retro raids at present and last night I got two (!!) epic tailoring recipes in COT Hyjal. I suppose that is payment for never (ever ever) getting the drake from Sarth 3D.

I love the idea of profession-based titles, though there's no way I'd try to take the title of "High Tinker" from Mekkatorque. You just don't mess with someone that short with that many inventions. ^_^
I'd also like to see class-based titles, perhaps based on spec - something you do a difficult quest for, one you *have* to do solo, and one that really makes you use your class's abilities to their fullest. For example:
Death Knight: the Frozen, the Bloody-Handed, the Necromancer
Warlock: the Plaguebringer, the Destroyer, the Demon-Binder
Hunter: Beastfriend, the Sniper, the Survivalist
Mage: the Flamewielder, the Arcanist, the Abominable
Paladin: the Guardian, the Lightwielder, the Healing Hand
Priest: the Sanctified, the Shadowcaster, the Disciple
Warrior: the Shieldbearer, the Weaponmaster, the Barbarian
Druid: of the Claw, the Preserver, of the Eclipse
Rogue: the Hidden Blade, the Unseen, the Swashbuckler
Shaman: the Earthwielder, the Lifegiver, the Stormcaller

Surgeon ranked higher than doctor? That's inacceptable. The surgical profession evolved from mere barbers, while physicians had an academic background (hence the "doctor").
Also, unlike nurse or doctor, no one was ever adressed as Surgeon Smith. It's his profession (just like doctor corresponds to physician).
The proper address would be Mr Smith, which to my knowledge is still the case for fellows of the Royal College of Surgeons.

Also, so long as there's "Dr. Phil," I really think the title of "Doctor" has been a bit devalued.
You make a good point about "Surgeon" not being a title, per se... but switch it around to "the Surgeon" and it becomes a lot more appealing for precise killer-type characters. And really, that's what it's about. Giving players fun ways to distinguish their characters. Not for real-world accuracy.

How about "Medic"-> "Nurse"-> "Doctor"?
Or alternatively, "Surgeon's Mate", "The Surgeon", "Surgeon General"

@Samutz... The only problem with that is that then you are relying on being grouped for 100 randoms with people that have also mastered their spec.
I do like the idea of spec based titles though. I'd like to throw out some other ideas for rogue spec titles. You guys can decide if they should be pre or post name, and obviously the exact wording would be adjusted accordingly (like adding 'of the' if the title comes after the name):
The Mutilator
The Assassin
Of the Furious Blades
On a Killing Spree
The Shadow Stepper
The Shadow Dancer
Just a few more ideas

The speleologist.
A title that actually would keep people within the dungeon, not having them drop out after the first trash wipe.
(Disclaimer: A number of pugs I've been involved with has actually finished it, even on normal. Once, we even had the same tank!)

I would very much like more profession titles, say for a certain percentage of recipes known or something.
I'm realm first Grand Master and Illustrious engineer and have every engineering recipe (except for goblin ones) in the game.
