I thought the Titans-- on their ongoing quest to structure planets-- found Azeroth next on their journey. When they got there, they encountered the elementals whose allegiance was bound to the Old Gods. Titans didn't like the Old Gods because they just seemed to be beings of "unfathomable evil," whatever that means. So the Titans went to war with the Elementals and the Old Gods, etc etc, chained and imprisoned the Old Gods when the Elementals were defeated, etc etc, then the Titans went to work on Azeroth, and voila! Azeroth as we know it!
That's kinda how I read the bit about "The Ordering of Azeroth", anyway.

I've actually never heard the story where the Titans came first, then the Old Gods, then the Titans came back to spank some serious tentacle behind. Anyone got a source for it? I'm really really curious now.
