If somehow a stack doesn't drop after a Hurl Spear on either tank, I make absolutely sure that on the next Hurl Spear ability if focus purely on constant MindFlaying Riplimb. It's almost a guaranteed stack reset. Sure, your dps might be a bit lower if your DoTs fall off, but your tank and healers will appreciate it.
