Replace Moonkin Form with Lazerchicken Aura and we're all good. You still have to click it on and it gives the buff without the model change. Then add Glyph of Lazerchicken as a minor glyph that changes your form to a moonkin as another effect from Lazerchicken Aura. Those that want the form get to keep it while those that don't want it don't have it.
Disclaimer: I don't actually think it should be called Lazerchicken Aura.

I'd be fine with a minor glyph that restores moonkin form (which I totally love and is why I play balance druids.)
Have to say that for herbing and questing, I do use flight form. For just exploring or wasting time, I usually use my black drake. It was my first time healing a raid, I won the roll on an insane number (like 97 or 98), and hell yeah I'm strutting that baby around. Plus, it's just too much fun to fly around the Twilight Highlands (or Coldarra or Blade's Edge Mountains) on a black dragon.

I say tie all the druid combat forms to a cooldown for each spec so they get used regularly but aren't permanent, then add minor glyphs for each spec that make them permanent.

I agree with those saying make it a glyph.. though I would argue a glyph to -remove- it. But PLEASE don't make it a CD! My boyfriend is still mourning how he only gets to be a tree briefly. I like being a moonkin. It's part of my identity as a character. If someone really wants it gone, then they should have that right. I just don't like them making the choice to take it away from me. And I AM excited about mogging. I already am farming the last of my leather set out of Karazhan. It's still fine for when I'm not in form. :)

I could see someone arguing that this makes it hard to differentiate between resto and balance in pvp, although it seems to work for resto/elemental shaman... Blizzard could always recolour the pvp ear for each tree I guess.

I'm with Anteia. If there's a glyph for it, it should be Glyph-to-Remove rather than Remove-and-Glyph-to-Replace.
My personal preference would be that, along with giving the Moonkin the Cat/Bear treatment of an updated skin and a customizable color palate.
