Basically you never want to have both of your blood runes on CD at the same time. Since that could mean wasting a RE proc on a worthless blood rune. So instead you have to make sure to only refresh it at the very end as the other blood rune is coming off CD. This is something you do to maximise the amount of DS you are able to cram out. Oh, and you should ever only use blood boil to refresh Blade Barrier. Since HS and BS can miss and thus not refreshing Blade Barrier. So yeah...
