Around Azeroth: The triage system is not infallible
While working on the Battlefront Triage quest, Starsmasher of Tempting Fate on Khaz Modan (US-H) ran into this Injured Earthen pinned under a pillar. Judging by the mechanism of injury, I would say that triage in this case is somewhat unnecessary; even though the Earthen is made of stone, that pillar looks a heck of a lot more solid. Pull out a black toe tag and move on to someone viable, Starsmasher!

Quested through that part of the zone on my Sunwalker yesterday. This isn't the weirdest thing you can do there, try aggroing a Needlerock Rider on a flying mount, reset him by flying up, repeat :D

Isn't that a catapult wheel not a pillar?
