The time me and my guild were running Halls of Reflection for the first time; nobody knew anything about the instance, we were figuring things out as we encountered it. Just defeated first boss and were running up the ramp, I was the tank, here's my thoughts, in order:

Oh, phew, there's Jaina, thought we lost her.
Uh, her health is kinda---
Oh crap it's the Lich King!
Uh... Jaina's dying... Time to panic?
No, I'm the tank, I'm a righteous paladin of the light, I will save her!

At which point I threw my shield in the Lich King's face.

I think by that point in Wrath, we all would have thrown our shields in his face. Even those who can't use 'em.
