Concerning the Custom options:To be fair, the choices for male body sizes are pretty boring. Granted it's more than you receive in WOW, but it's limited to tiny, less tiny, bulky, and fat dude. And as TOR stands, all of the races are humanoid. So they merely changed around slight facial differences for every race and changed the social ability. At least with WOW the race option was much more of a choice. Hopefully they fix that in the future.

A) I found the male body options to be skinny, normal, He-Man, and meat mountain. The last one is bulky as anything, but I don't really think 'fat' applies, at least not in the jiggling rolls sort of way.B) The NPCs do react differently to your race, at least a little. Although, admittedly, most of that reaction was along merely the human/not-human axis. Well, by 'react,' I mean the dialogue changes. Not so much the outcome of the encounter; by an amazing coincidence, despite how speciesist the Sith Empire is, all the imperial questgivers are able to look past your race and see how well you perform.

This was one of the few things Aion really did get right. The sheer wealth of customization options for your character's appearance was astonishing. I was literally able to make one character modeled after Brock Samson and one character modeled after Master Billy Quizboy, one character modeled after Buffie the Body and one character modeled after Rukia Kuchki (or however you spell that). Sure, it's there to compensate for the fact that they only have two races, but it's an outstanding feature and it really helps you feel ownership over and connection to your character.
