That looks like a replica of shaman tier 2

I think the shoulders are Mantle of the Sea Wolf

and the head is possibly Headdress of the Tides
http://www.wowhead.com/item=27759

Shoulders drops from heroic Tavarok in Mana-Tombs and the head drops from heroic Ghaz'an in The Underbog.

When I said Mantle of the Sea Wolf, I of course meant Pauldrons of the Wild Magic. Keeping track of all these tabs and names is hard when raiding. :3
