You may be able to get some of this information (at least circumstantially) via Wowprogress. Try entering:

http://www.wowprogress.com/character/us/blackrock/Rushift

Except replace your region/realm/Charactername as appropriate. Down at the bottom will be a bit of history.

You can also go to http://www.wowcensus.com/charhistory.php and enter your information there (you'll have to search for any previous names/servers/etc. separately). It has most of my main character's history there.
