People hit their /roll button and then quickly drop group before it finishes choosing a winner. The loot doesn't always follow them back to their home server. It's just too darn hard to wait a few seconds, y'know. This is what I most commonly see, anyway.

This thing did happen to my roommate however, where she saw she won the roll and *then* dropped group/left dungeon, but the item still didn't auto-loot to her bags after she had won and she couldn't get back in. That was weeks ago, however.
