There are three types of "nerfs."
Type A: Minor adjustments. These are usually made within the first month or two, and are usually fixing mistakes or smoothing over bits that were overtuned. For example, when they changed the bird boss's debuff in ZA in 4.1 so that it was possible to do that encounter with a melee-heavy party, or when they reduced (IIRC) the HP of the burning tendons in H Spine because the encounter was impossible to clear unless you stacked high-burst DPS and multiple Dragonwraths. This is fine.
Type B: Optional buffs/debuffs, like the one in ICC or the current one in Cata. Being optional, this is fine-- you can turn it off if you want the original difficulty.
Type C: Harsh blanket nerfs, like in Firelands. This should not happen on heroic modes. The only time it might be acceptable is for times like the BC-Wrath transition, when they honestly thought the stat changes would make the raids impossible, and even then only to even things out. For example, if they do the item squish they will need to rebalance everything to fit the new numbers.
